New customer database thats drives customer reports

Hi I need a Customer database designed. The database will need to drive customer driven reports and internal reports. It will have to be designed so i can add worksheets / Fields To be able to be used on different PCs, via sharepoints or similar? First element is a standard CRM, input customer details etc That CRM data will then be able to be used to carry out inspection process reports with multiple fields and variables (See attachment for final report) The data entry should be able to select and add different parameters for different equipment. i.e. Equipment 1 - requires Test 1, Test2, Test3, Test4 Equipment 2 - requires Test2, Test3, Test5 These should then save the data input for that model as a template for further inputs.
