I need a online form for my site, was derailed by previous programmer although he almost finished it, one of the forms work and the other one does not. you can find it here: [login to view URL] [login to view URL] I need these forms re-built in a correct way. I would prefer it to be tied into woocommerce which is currently set-up and connected to the theme. The form will calculate a price and pass it along to paypal.
