I can very easily create a one-page website hosted on a free source which would be able to collect a number of answers and add them to a database using PHP and MySQL. Depending on how important styling is, my knowledge of CSS and HTML5 could help to make the form visually appealing.

I have developed a similar project in my previous project. Main form was input form and storage was Google spreadsheet. Even there was searching functionality to view a particular record in pdf format.
