Service order form for a Wordpress website

Company formation order form based on gravity forms WordPress plugin The site is based on WordPress with Avada theme We have an existing working order form for ordering offshore company formation with optional bank account Link to the working form and more information can be found here: [login to view URL] For a new site, we need a form with similar functionality to order company formation (without ordering bank account ), based on Gravity forms plugin Multi stage form (number of stage will be decided later) Nice high-tech design (based on gravity forms options and Avada theme options) Note: * Custom fields need to be added to post or post type as needed * Service offer pages (e.g. company formation in country XXXXX) will have an order button which will pass initial order information to the order form (see the existing site for example) * user might want to go back in form stages to edit details * the side widget displaying order information * The form need to be responsive * During all stages of development and testing, an up-to-date version of the code deed to be maintained on our server (i.e. on our test site) * Site will be multi language (based on WPML), Form must be localisation ready (use .po language files), and support language direction (RTL, LTR WordPress) * service provider must test his work, I will not do the QA !!
