Additional data fields based on drop down selection

On the recipient information page, users have the ability to select home, apartment, business, church, funeral. Project: when a user selects church or funeral, a new drop down and text field appears below. The first drop down is called "Service Time:" and let's the user select: 0:00, 1:00, 2:00 through to 24:00. The text field is called "In memory of:" This information should be carried to the admin panel, so when a user looks up order numbers, they can see this data.
