data entry apps using android - repost (upgrade - phase 2)

hello, the apps has specifications as follows: 1. user fill in the answer sheet form (textbox, checkbox or radio button) 2. send data via internet to mysql server (included gps info, imagess, audio files) 3. administrator can see all user activities via php web apps 4. administrator can change the answer sheet form remotely. 5. if internet connection broken, the data store locally on device, and send automatically, if internet connection goes on.
