Acrobat forms

I need some documents converted to Adobe Acrobat Form. I have a small website/company specializing in software taxation issues. See www.softtax.com. I need some current HTML documents converted to Adobe Acrobat Intelligent Forms that can be filled out on screen and then printed out. (Is this called an intelligent form). The two forms I want to do now: 1. [login to view URL] 2. [login to view URL] opens up. Instead of just having an HTML form open up, I want to give my clients the option of opening up a PDF form that can be filled out on screen, and then printed out and faxed to me. Requirements: 1. The order form must be no more than one page. 2. Some design work is necessary. I need the following added to the forms. a. An entry for the card verification (four digits) b. The name on the card if it differs from the person in whose name the order is placed. (e.g., the card may be in the purchasing dept's name, but the order goes to the CFO); c. The billing address if it is different from the shipping address; and d, Move the identifying material (my name and phone numbers) and instructions relocated from the bottom of the page to the top. I have some basic ideas as to fonts (all rather standard I think) which I will discuss with the designer. Please do not make an offer to have the client fill out the form and electronically sent it to me. As stated above, I require each form be signed. The form must be mailed or faxed to me. I am a Mac user and have Acrobat 7.0 professional for the Mac. What ever you provide should work with all current versions of Acrobat on all web browsers (provided the have the acrobat plug-in). ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform needs to run on all major web browsers that have the current acrobat plugins
