Adobe Acrobat fillable form

I am looking for someone well versed in Adobe Acrobat forms. What I am looking for will be pretty simple for this person. I will have a long form where our clients will fill in the first page with information. I then want that information to populate the rest of the form where ever it should go. I am finishing putting together the documents in electonic format. I should have that done by late next week (by Oct. 7). This will be a static form. Meaning nothing will be emailed or sent via internet. We plan on sending this form out to the clients via cd. The form should be usable to anyone with just adobe reader 6.0 or newer. I do have a deadline where I need a functionalbe form for a presentation by October 13th. This does not have to be the final product, but something close so I can demonstrate. We will probably want to make some updates down the line so if done write good possiblity for addtional business for the coder. The final project should work like this. 1. We send client the acrobat form via email or cd. 2 The fill in the form fields on the first page, These then populate the rest of the document. 3. The client then prints, signs and sends back to us. Hopefully I have made that as clear as possible ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Final product must work with Adobe Acrobat Reader 6.0 or higher. This will be used only on Windows based machines.
