ASP or PHP based survey tool

Need a pretty standard ASP or PHP-based survey front-end developed, based on a set of stored procedure procedure calls to retrieve survey items, and another to save the item data. In my datamodel a survey consists of a certain number of items, and each item can be of six different types: Header (display only), Date, Quantity, Multiple-choice, Text and Calculated Display only). The user logs in and creates a session, where a session id is stored. Next, a survey is selected from a list of available items. The data for a survey (form) is retrieved with three stored procedure calls: GetSurveyEnumerations( SurveyId, LanguageId ) GetSurveyItems( SurveyId, LanguageId ); GetSurveyConditions( SurveyId ) An Enumeration here is just a list of multiple choice items previously defined for reuse, and zero or more enumerations. The survey items should be displayed one at a time, optimized for 240x320 screen size (PDA). A survey item in the sequence can be visible or invisible, based on the value of previous items (branching), including previously calculated values (which must be integers if they are to be used for branching). To save any item, a stored procedure is called, based on the item type: SaveQuantityData( SurveyId, SessionId, VariableName, FloatValue ) SaveOrdinalData( SurveyId, SessionId, VariableName, IntegerValue ) SaveTextData( SurveyId, SessionId, VariableName, TextValue) SaveDateData( SurveyId, SessionId, VariableName, DateValue) Most of this is pretty standard stuff available in all survey software, so the special needs for this project is the calculated items and the interfacing to an exisiting database for retrieving survey information and storing the results. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform PHP or ASP
