We have piloted the CRF you built for us and based upon feedback from some doctors and researchers I wanted to modify the CRF field choices and also transfer the platform from Excel to Access (my team now have Office365 now). The changes to the original CRF include addition of a couple of BASELINE data fields and HISTORICAL data fields. The field researchers need an Access form for data entry. Some data for the FORM will be pre-populated or drop-down options. All data should eventually be available for analysis in a mastertable.

Based on your data sheet this is a straight forward MS Access application. You will need a primary data entry form, possibly an edit form to view and/or edit previous historical records. The data will be organized with several individual tables holding your drop down information, one table to hold your customer information, and one main data table with all of your historical visit records. If you know what kind of reporting you need, we can design a separate reporting screen with reports generated in either Excel or PDF format based on your visit data. I have over 8 years of experience in transferring data from excel to an access database. as well as making that database unique to your company. I have taken a look at the basics of your excel sheet provided and the information is very straight forward. the drop down menus for me are the simplest part of this since forms and other things required just build off of themselves. please contact me for information on the template that I started building based on what you have already provided.
