The attached pdf is a copy of our patient report. I need someone today to redesign this as we only have this low res PDF copy. - design EXACTLY the same form using existing fonts of MENTONE ( i can supply this font) & EUROSTILE - logo will be supplied but the IMAGE of the human body is not (this will need to be redrawn) - the size is A4 landscape - There will be one round of changes (my comments) that will need to be made - The red number at the bottom right of the form ( 11,222) needs to be deleted and left blank for printing - Keep colours, fonts, capitals etc the same as the JPG - supply high res print ready PDF with crop and bleeds - supply all files (adobe etc) with fonts and images embedded etc. I need this TODAY - if you can't do this TODAY exactly as brief DO NOT RESPOND
