Online Survey with Date Calculation and more

I need someone to develop a type of online questionnaire that allows clients to answer a few questions about their current credit situation. Then I want there to be an analysis of a certain date that would be estimated for home loan qualification based on those answers. I want this date to be given online immediately once they submit the answers. I then would like (after they submit their personal info) it to be able to give a general analysis of how we came up with that date with specifics on what we suggest they do to get to that goal date. This would preferably be in email form. I of course will supply all questions, and the corresponding timeline for each answer. Ultimately the date will simply be the longest waiting period of all the answers and so this calc is easy. Here is an example: 3 Questions: 1. Do you have new credit reporting on time and current on your credit report? a. yes 1a. How many months has that credit been established? (example answer) 4 months. This would auto set the date at 6 months from todays date so that a total of 10 months re-established credit is met b. no This would automatically put the timeline 10 months from today's date as new credit is required for the loan. 2. Do you have any federal debts currently in default such as student loans or FHA mortgages? a. yes This would auto set the date at 12 months from today's date so that the requirement for 1 year fed debt timely payment is met b. no No change to date from question #1 3. Do you have any judgments, state or federal tax liens on your credit report? a. yes 1a. How long would it take for you to payoff/settle these debts? The amount of time they put here would be a factor to question 1 only if the time period is longer b. no No further change to time period determined in questions 1 and 2 I am no engineer and there may be an easier way to structure this but basically it is an "if so, then.." type of questionnaire. From here I would like there to be some pre-scripted info that would explain how we based the factors. For instance if in question number 2 the client answered yes it would bring up this factor in the explanation that would be prewritten and explains that federal debts have to be paid timely for 12 months in order to obtain a home loan. If they answered no, this factor would not be in the email explanation. I would like this project to be licensed only to me. We have a very specific type of financing and this should not be made available to others under any circumstance.
