Google Form -- 2

We are using a google form for conferences (branches) to upload their yearly activities. We need a searchable dropdown box where they can select from a pre populated list the area the conference/branch belongs to. Depending on which area they select, they must then select the name of their conference/branch from a pre populated list of conferences/branches for that area. There are up to 40 different options for each of these two questions so the dropdown box needs to be searchable - for instance the use can type "uni" and would get options for "United Kingdom" or "United States"; Each conference/branch can only submit once. Once the area and conference/branch is selected, there are 22 standard questions that must be completed - including email address and telephone number which must be in the correct formats. The form must have the organisation logo and name - "Society of St Vincent de Paul South Africa" and use the organisation colours
