Do some Excel Work

We are looking for someone in Excel to create staff monitoring sheets for a firm of accountants. The staff will complete these monthly and will self-evaluate and then send to Directors for review and comments. The idea is that they take no more than 10/15 minutes to complete and highlight how well they have done in agreed objectives. There is white space for them to then comment and overall Directors comments for that month. In the attached snapshot it is an example but actually there will be at least 20 objectives and more free space than currently shown. OTHER:- • should be ease of completion • have our logo and design • each monthly submission be capable of being saved • TWO forms required as templates – one for Admin staff and one for PROFESSIONAL staff • Capable of being amended • If they can be linked ie each monthly to a yearly summary re scores that would be good • Helps staff self monitor progress and identify training needs AS WELL as pushes them to achieve more and cross sell to clients and FOCUS on revenue NEED:- Competitive quote to do this exercise in excel. Fixed price for whole project including templates, we have control over changing, designed well and have easy completion use by clicking. Dashboard of scores for each month.
