Fill in a Spreadsheet with Data

Using the Web Survey free trial account from [login to view URL] to win the entire project you have to demonstrate that you are able to migrate at least 10 questions and answers of different types and categories (Multiple Choice, Radio Button, Drop Downs, Rating Scale, Numeric Input, Text Input, etc.) from the Health Risk Assessment Survey located at http://ThePreventionPlan.com. Then if you win the project, you will use my developers account at Select Survey to enter all 100+ questions from [login to view URL] including the Categories for every group of questions in that entire Health Risk Survey. The Data Entry is done into an Excel type Web Form using Point and Click interfaces. If any questions have typos or missing choices or if they used the wrong Question or Answer Types you will not be paid for that question. I expect that the bids should be in the range of about $2.00 to $3.00 per question or between $200.00 USD and $300.00 USD in total. It should be entirely completed within 7 days of award of the project.
