Build a WordPress plugin for a Form and Email interface that let forms be send to different e-mail addresses according by the choices they make in the form

WordPress Form and Email interface / plugin WordPress Form and Email interface that let forms be send to different e-mail addresses according by the choices they make in the form. There is 1 field in the form that has city names in it. (Amsterdam / New York etc). There are a fixed number of cities in the list (approx. 500). When the user fills in the form he chooses 1 of those cities. The output from the form will be send to the e-mailaddress connected to that city (1 or multiple e-mailadresses). Cities and emailaddresses are connected by he admin in the Admin interface pages subscribed below. Examples: When in the form the city Amsterdam is checked, the mail goes to amsterdam@[login to view URL] When checked London the mails goes to: londonoffice@[login to view URL] The interface / plugin has multiple interface pages. FORM Page This page hold the form that visitors from the site can fill in. It’s a normal contact form like any other with some product information. Name address city phone email and other fields Admin interface page 1: The admin Interface page lets the admin: - add new users - delete users - change users - add e-mailadresses Admin interface page 2: When the admin clicks on a user on admin interface page 1 The admin goes to interface page 2 where he can appoint 1 or more of the 500 cities in the list. This page displays all the cities. The admin can check the boxes of the cities he wants to appoint. Admin interface page 3: Here the admin sees a oversight of all forms that where filled in and send to the different emailadresses. The admin can RESEND the mails by clicking on the RESEND button you make. User interface page 1: The user can do the following actions - See all the forms send to his account and has a RESEND button like in the admin page. This WordPress plugin needs to be able to be used on other sites as well. You can make your own form and send/mail option or you can use Gravity Forms to work with is plugin. Both are acceptable for us. 50% of the bid is set as a milestone and released when the plugin is ready for us to test. When finetuning is done and we did test the plugin for 3 days the last milestone is made / released. When you start you will get the list with the 500 cities and access to a website where you can build the plugin online.
