I've developed a few MediaWiki skins before ([login to view URL] and [login to view URL]), and I've had to deal with the WikiHow mod of MediaWiki in a previous job. As you may already know, the WikiHow software is a heavily modified fork of a quite old version of MediaWiki (1.12, the current one is 1.22), so adapting their skin to the current version of MediaWiki will be a many-hour job. But I can definitely do it. Also, some of the elements you see at WikiHow do not come from the skin, but rather from some of the many extensions they have installed ([login to view URL]:Version). Some of these extensions are standard, others are unique to WikiHow, or unique modifications of standard extensions. Thus if you want some elements to be part of your wiki, it may be necessary to adapt not only the skin, but some of the extensions too. I can also do that.
Another approach is to start a skin from scratch, based on your directives of the elements that you definitely want to have in your wiki. This we would need to talk in detail, but again, I'm definitely able to do it!
