Create PowerPoint Slide With Editable Content from PDF example

I have an example PDF document which I need converted into a editable Microsoft Powerpoint slide. The following are the specific requirements:
* PDF source document attached
* Don't need techsecure holdings name, graphic, title or document classified for public access included, all of those can be left out
* All of the boxes and text around the boxes in a powerpoint side
* All the information rotated 90 degrees to it fits in a powerpoint slide in landscape format (currently in portrait format)
* All of the information int eh boxes and box labels must fit in a standard US letter landscape powerpoint side
* Boxes should be flat (not be 3d / shadowed as original)
* All boxes and text needs to be editable in powerpoint
* Formatted for Microsoft PowerPoint 2010 in pptx
