Insert some extra choise radio buttons in website

Hi,
I'm looking for a programmer that can delete the current radio buttons and replace it for an dropdown.
With 2 extra function,
1) A way that i can insert extra more send rules from the [login to view URL] file.
Default in the site there is define only an SMS gateway as API but i want to simply extract this with:
- and maybe more
(Mail and URL is default build in the website and is send direct to the Mysql db)
2) When the user is select for example the SMS in the input rule the user see the description what they must insert there.
SMS = Phone number
Twitter = Twitter name
Facebook = Facebook name
Mail = Mail adres
Etc
