Create a custom wordpress form plugin based on existing script

I would like to create a WordPress plugin based off of the following script: [login to view URL] The plugin will have an admin control, where you can create forms (just text input for now) and change colors, fonts (google fonts) etc on each form. The final design will look and function very similar to this form: [login to view URL] but I do not want to use this script as it is buggy and is not flexible for future development. It will be exactly like that form, but it will also have circle pagination which you can go BACK to previous fields. See attachments. I already have a HTML template created for the email that will be sent when the form is completed. The form should use basic WP Mail to send the email. I have attached mockups of how the front end and back end should look.
