479185 Raw Food logo

I need a logo header for my blog. I have a 90 Day Raw Food Challenge on Facebook and I am starting a blog. I want a background picture of a bounty of fruit, (I already have a photo) I want Pamm Turner Body Coach over the fruit in a raised, shadowed font. (You can be creative with this, but I want it feminine. I would also consider a frosted transparent, beveled font where I can see the fruit underneath!) I want "Pamm Turner" in a pretty script font. Please skew my name across the top counter clockwise. I want "Body Coach" in a regular font. Please position straight along the bottom more to the right of the Header. If the font is not the frosted transparent, then please have the lettering in white. In the left corner I want my photo, which I will provide, over the fruit. I am attaching a file my son did for me as an example. He is a kid so please just use this as a sample. I would like it more professional.
