Online Weight & Body Measurements Tracker

I am wanting a weight tracker inside the client portal for each patient to be able to monitor their progress. It will need to be a graph (line or bar) which will give a visual representation to their status. The y-axis will be the dates they enter the data and the x-axis will be their weight. The second graph will be similar but will have their measurements of key areas (waist, arms, etc) all on the one graph to continue to show progress.
