Create a User Friendly Health Proposal Template

We are a health insurance agency and we need a few changes made to our health proposal template to make it more user friendly for our sales agents. The formatting from tab-to-tab is always out of whack, we need this to be standardized and locked down to prevent too much alteration. We need to be able to change the plans (add/delete) and the figures associated with those plans. Just for your information, these plans only typically change every two years and there are only a certain number of plans available by carrier. The formulas also need to be corrected to accommodate the 4 types of ways someone can get paid: weekly, bi-weekly, semi-monthly and monthly. Deductions need to be calculated either as a percentage of some number or a flat dollar amount. Typically, the employer only pays for the employee only rate, so all other deductions need to be calculated net the employer contribution for that employee. The number of plans by carrier can vary. We need to be able to put up to 5 plans for one carrier.

Attached is an example of what fields need to be fixed, and the spreadsheet we are currently using that is not user friendly. We need a user-friendly solution completed as soon as possible.
