TO CLARIFY*: I think my initial instructions made the $37 add on seem to be only for the "Membership". Really, the $37 add on would apply to all categories. For example, if someone did the non-member $65 adjustment and wanted to get Shockwave also, the shockwave would be charged at the $37 add on price.

Need a price menu to be used at checkout. This is for the front desk sales person to present our membership options to the patients at a chiropractic office.
