Help me build an custom HTML for shopify theme to ask certain questions to a visitor & suggest product based on that & mail me those answers so I can know my visitors better.

I want customers to get suggestions on based on their answers. I want them to answer a few questions of mine the moment they visit my website so I can know them a bit more about them & collect their emails. So build can custom HTML with very good looks & with a gif behind every relative question. I want to ask their fav. colors ( multiple can be selected), I want to ask their sizes of Sweater & Pants, I want to ask them if they would wear products purchased from my website at home, work or party. I want to ask them if they love to have pockets in their every cloth like sweaters, pants, shirts, t shirts, and so on. I want to know how do they came to know about my website. I want to know where they would go next after this website, lastly I would like to know if they would love to try any one of my product for free? & then ask for their name , age sex, & email. & also I would love to ask them certain personality test questions. I dont have gif files yet, I will arrange them later. But Do give me freedom so I can change the order of questions, change questions & answers to them & also delete some questions in order to test what works and what doesn't.
