Fix my subscribe form

I have an existing subscription form where a user can subscribe to free fare alerts.

Now this form is flawed and needs to be improved:
- the whole airport list is now static but it should come from database dynamically (MySQL) and not when page loads (as it is done now)
- the form behaves badly with some browsers - it needs to be tested and stable with all major browsers and devices
- sometimes no airport is found in our list - we then allow a 'free-text' input - that is ok but we should minimize this as it requires manual rework
