OS Commerce - Need Product Attributes text box

Greetings Freelancers We Need the following contribution (or similar) added to an OSC 2.2 website The shop is in construction and is pretty much done except for 2-3 modules left to install and enhancement of graphics. The compagny is called kehni. Kehni is a small shop that manufacture towels, wash cloth, bathrobe, sheets, etc The attributes text box is needed to take sheets mesure from a client. The client will enter it's table dimensions in the text box, which will be logged into the order or the quote. The text field box will be used along side the standard select attributes method. I need the ability to have several text box attributes per product. It will be installed of 3 or 4 products. The informations will be gathered from the text box and should be sent to the store admin along with the order details just as normal attributes would. I will also need any documentation so I can add this modification to other installs if needed. Here is a link to the existing contribution. I am unsure if this works as is with STS at this time.
