php mysql database

I need help creating php app for my php database. very basic but must be done immediately - hopefully sunday/monday. we have 4 tables: 1) location (32) 2) sign descriptions (can be many signs at one location) (40) 3) location activity log - can be many activities for each location (4) 4) sign activity log - can be many activities for each sign (4) (number of fields in each shown) location data screen has typical navigation - move forward, backward, add new record, show signs at this location, add sign to this location sign data screen shows all signs at active location. usually 1-4 signs at a location. nearly all fields on tables (1) and (2) will have drop down lists. must have an easy way to add items to the dropdown list. tables (1) and (2) will need to have 1 thumbnail image and 1 photo stored and displayed with each record data entry screens for (1) & (2) will each have a button to "SHOW CATALOG ITEMS" - this will display a gallery view of the thumbnails. when a user selects a thumbnail, he will have 3 choices - CANCEL, CLONE ALL FIELDS, CLONE TO BLANK FIELDS - the catalog data will be used to populate the same fields of the current record. rough mockup of data entry screens attached. screens need to be optimized for ipad/droid viewing. i've already created the table definitions in phpMyAdmin If you are ready to work immediately, please bid.
