Flash intro plus website (all one page)

I am looking for three different parts 1) I need a flash presentation/intro that displays our product shot in a very inique and eye opening way. Check out the intro for [login to view URL] to see a good example (look at there intro) 2) The flash presentation and image shots then need to be integrated with a order form for the products. This all needs to be one page, so at the top or the opener they see the flash presenation then they can scroll down and see each shirt and pick which ones they want. 3) the last part is they then need to be able to write in there orders and information and have it submit to me. This all needs to be one page and very simple and easy to follow. Please do not bid if you do not understand what I am looking for or you are creative.
