Mobile development

I need an iPhone/iPad app. I would like it designed and built. I currently use a paper lead management system that is inefficient. I have to file paper in folders and no real good way to track progress or follow up. The source data is a pdf. lead card, and a csv. with the same information. I use the pdf. card on appointments, and the csv. file for tracking. My progress notes are written on the back of the card and annotated on the csv file. Neither is very good. I need a system where I can upload the csv file and attache the card to the file, so I can toggle from the dashboard to the pdf. I need to be able to add notes to each individual file on progress and be able to sort the progress. There are only a few things that can be annotated. The date and time of contact, the type of contact: phone or door knock, the result which is: appointment scheduled ( and if an appointment is scheduled the data needs to go into the google calendar, and todo list), Voice mail (no contact), call back at a certain date or time, also to go into calendar, and a todo list. and a call that is rejected or no advance. So you get an appointment, or have to call back because they are not in, don't answer, or want to speak at a different time, or no advance and the lead is no good. Some leads do not have phone numbers so I am going to have to door knock to contact them. If you get an appointment the lead is either sold or not, there may be a reason to call back later in a month or so. You can also get a no show appointment. Sale, No Sale, or no show, and comments for all. I need to be able to sort the data. So when I get the leads, I will sort into doors to be knocked and calls to be made that week. I need to know who I need to follow up from prior calls or visits, and I need to be able to sort by zip code for efficiency. I need to be able to access the data on my ipad, and make the notes after each call, sold or not sold, and if I will follow up or not. I will need to sort data by any field so I may want to know how many how many people no showed last year, or in a certain town, I need to be able to work the data on a dash board, so when a lead is pulled up I will have all the data on that lead available, and all the contacts and notes available without further searches. If the lead is sold I need to be able to record the sale amount. 150 , or 600 or whatever. So I will know what my sales progress is for the day or week or month. I am attaching examples of lead cards and csv file, and some notes on the back for examples. My ultimate goal is to put this into an ipad app that will be able to access data from a cloud source, I will need to be able to print out a todo list of who to call, and it would be nice if the appointments and follow ups would be able to go into google calendar. Each call or contact on a lead is a separate event that needs to be in a history so it can be recalled.
