Ronald McDonald House Cookie Patrol Postcards

I am on the Board for the San Antonio Ronald McDonald House Charity. I was asked to help with a task where volunteers are mailed post cards to those who have volunteered previously to donate food items to the four houses in San Antonio. The previous design of the cookie patrol postcard is attached as a picture. Your task would be to modify the previous design in the following manner: Old Text: * Downtown Location 227 Lewis Street 223-6014 * North Side Location 4803 Sid Katz 614-2554 New Text: * Downtown Location 227 Lewis Street (210) 223-6014 * North Side Location 4803 Sid Katz (210) 614-2554 * Med Center 4847 Charles Katz Ph - TBD * Children's Hospital 333 North Santa Rosa (210) 704-2383 You must recreate the document from the attached image pdf. The original document has been misplaced on the charity network or lost due to outgoing volunteer staff. The finished product for this task is to provide me with two pdf forms, or a two page duplex form. One for each side of the postcards. One side will contain the existing design with the changes made to the text, and this side will also include fields in the form suitable that I can develop a mail merge process for the form. Where you see blanks on the original, they should become text boxes in the PDF form. The other side will be used for mailing the post card so it will include this address as the return address Ronald McDonald House 4803 Sid Katz Dr San Antonio, TX 78229 and contain mailing fields (US style address) to mail cards to recipients through a mail merge process. Example layout would be: NAME AddressLine1 AddressLine2 City, State Zip The workflow would be to print approximately four cards on one side of the paper, then flip the paper over and print the addresses on the other side. Or, the pdf form(s) could be in two pages such that if printed in duplex they would print exactly correct for cutting out and mailing.
