Headed Paper Word Template

I currently have a business logo, headed paper and report template. I have two requirements for this project: One - the headed paper design I have for the word document has the design inserted as a background. This means when I move onto the second page, it repeats the background design. This does not look good. I require a word template where the first and second and subsequent pages are different. The first page should have the design header and footer as is designed in the original headed paper. The second and subsequent pages should just have the footer. I will be pdf'ing all these documents so I require this document to be fully editable in word and to have an editable space suitable to the headed paper design. I require one word template document which is fully editable. Two - I require a fully editable word report document which has a designed editable front page similar to the report file attached. I then require a second page which has an automatically updated index followed by a third page which has formatted headings, sub headings etc. I then require a final page which contains my address details etc. I have copied the link to a similar design here [login to view URL] I require the headings and sub heading to be text and colour formatted in line with the stationary. I also require the body of the report to be able to be added to and where the index will automatically update as pages, and section headings are added. I require it to be fully formatted and in one word document. I therefore expect two fully editable word documents at the end of this project.
