Simple Customer Database

I need a simple customer Database that can be accessed by the office staff. this is not for customers but for internal use only.
1. It will require a web front end with secure password
2. It will not require an admin area
3. Ideally mysql should be that database back end
4. It should be designed to hold up to 5,000 unique customers
While I will provide more detailed requirements to the winning bidder, the essential idea is to track existing customers and their past orders and allow for some staff comments on each.
