What you have explained here is a Database driven platform where each user will have their very own private dashboard so that they can manage and streamline their activity within the platform as you mentioned . Each such user dashboard directly feeds in and out of the mother database . Of course the admin ( in this case I am assuming you) will have a MASTER-DASHBOARD to manage all the activity and every aspect of the platform and all the users from his / her dashboard. So therefore , based on these facts , I am considering that the database schema will indeed be an ORM DATABASE so that all the modules within the platform will be dynamic and fluid it grows as your user engagement grows . So the idea is to create a collaborative platform between the doctors where the users can self assess and monitor their intake and the doctors can keep themselves updated on the situation of each user. For the reminders , all notifications would be fired automatically based on the selected preference of each user which is managed from the dashboard. there are only a few more things that needs answering so that I can understand your deployment. So let me elaborate on that .
