We have an online form that our users uses to fill digital versions of our job safety analysis. However, we currently do not have a physical version of the form, so we would like to have designed an A4 type of form that users can print out.

The required form elements are the ones showed in the example form we created.  The «Hva kan gå galt / risikomomenter» and the «nødvendige sikkerhetstiltak» can have unlimited rows.

The signature part can have up to a maximum of 10 individual signatures with a name and date.

You can use some artistic freedom, but regarding colors, its advisable to mix in our brand color #0AAEF3 for styling. The design should look more professional than playful.
